OK, I may be rude sometimes, but some doctors are not only rude but downright idiots.
We have this one doc that is ok, sometimes fun, kind of a dork, you know he got beat up on his way home from school as a kid and his backpack thrown into a tree or dumpster, etc.
BUT, usually his 1st two days and his last two days of work ( he does a 2 week run then is off 2 weeks) he is a dick. condescending, speaks really slow to you and enunciates his words. you just want to slap the shit out of him. I tend to answer him the same way he talks to me, no matter which voice he is using. He gets confused tho and I think he really doesn't get it.
Anyway, last evening he is on one of his last 2 shifts (becomes stupid and rude), Oh and he also has this problem with the nurses starting care. In his first 2 shifts it is 'you know this is an emergency why didn't someone do this or that'' In his last 2 shifts, he likes to change whatever you did and question why you did it and order the opposite of what you did.
Well we are not a trauma center, I tell people all the time if you get hit by a car, go to -------, If you are having a heart attack, come to us.
So last night this guy walks in post welding next to a gas can (Darwin!) and guess what? It explodes. face looks like a really bad sunburn with pretty much all the hair missing from the inside of his nose, eyebrows etc, he now has what looks like male pattern baldness.
But both arms and hands all the way to the pits and circumfrential, are missing most of the skin, actually the skin was there just hanging in strips.
Well, maybe im old school, but i was taught to cover with sterile wet gauze. the pt was screaming to pour the water because it helped. so we gauzed him and poured normal saline on the gauze while waiting for the doc. Yes the pt was laying in a pool of water, which I already had the techs ready to clean up. This dipshit doc (see above) comes in and says stop your doing it wrong and walks out. so i follow. He tells me that is not how we do it. and i tell him I have been here longer than he has and we have always done the gauze water thing. (if he had gotten there first it would have been fine) no, no, no, he wants us to remove everything and place vaseline gauze with kerlex on the arms, I am fine with this if this has been proven more effective. but fuck you for embarrassing my nurses in front of the pt and his family. To which I told him as such. and getting the pain medication out of him was torture also. he only orders 1 mg of dilaudid. (because I asked him for 2mg) He gives the seekers more than that. burns fucking hurt. Migraines get 4 mg IM. etc. dickhead. so he orders it as 1mg q 10min, prn pain greater than 5----WTF---he has never order meds that way ever.
Anyway.....we take all the saline gauze off and gently wrap him in vaseline gauze with kerlex. and pull out 4 mg of dilaudid, so we are ready for the 10 fucking min.
In the meantime the doc is talking to the burn unit we are transfering him to. rule of nines he was around a 20 and in our hospital anything over 10% goes to a burn unit. Well they tell him what they want and he doesn't tell us. When my nurse calls to give report to receiving hospital, they tell my nurse that they want standing order for meds durning transfer (OK) AND SALINE SOAKED GAUZE!!!!!!!!!!!!!!!!So now we unwrap him again and put the saline gauze back on. all the while looking like idiots to the family. I don't usually do this but totally threw that doc under the bus with the family. They told him later how stupid it was to put that man thru that, and that maybe he should have listened to the nurses.
I went back to the nurses station and told him what kind of dickhead he was and to NEVER put me in that position again. the wrote up the unusual occurance form.
We normally stick together and if an md makes a poor decision, we come up with excuses for familoy, hey we all make mistakes.
No apology to us, no hey I was wrong. FUCKER!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

Had a great shift
So yesterday evening, we went LIVE with our new computer system. All the hospitals will eventually be connected. The system is EPIC and we call our end HEALTH CONNECT.
The powers that be have sent us to classes (basically worthless) . Outside of the basics, we got a lot of 'well you can click here for vital signs, but you won't use that, or click here for neuro, but you won't use that' we came out very frustrated. Then for the 2 weeks prior to go live yesterday, we got to 'play in the sandbox' there were computer people all over the place and the crew would take turns 'pooping in the sandbox' this was way more effective than the classes, as it was geared for us.
As background we do some computerized charting thru triage and the docs use the computer for d/c and h & p's, etc. The new system is a monster tho. Completely paperless. except codes.
The stress level was way high, so for the few days before going live, i decided to work on the stress level instead. I had a pool party for my shift people, continued everyday telling people not to worry. we had extra people and a few nurses that are considered experts.
I'm not really a ZEN person, but right before the shift, we decided that my peeps are going to kick some serious ass, and had a little huddle in the break room.
We walked out to the nurse's station to a cluster fuck of crazy. The day shift was running around telling us how horrible it was. they were rooming pts too fast. every time a room emptied and before the nurse had a chance to breathe the charge nurse was putting more people in.
Well im the charge on my shift. I told her stop rooming people, now, unless someone is truly sick. I was watching the circus and decided to change the way i did the assignments. Instead of 4 pts for each rn, i eliminated the float position and gave all the nurses 2 pts each. Then called another 'huddle' to remind them if they get stuck to just put up a hand and one of the experts will be there.
It worked beautiful, it took about an hour to calm everyone down. Then we had the smoothest go live. The pt load was good, as for breaks, the nurses tag teamed, and took care of someone elses other 2 and vice versa. My crew was helpful to each other. AND pt care didn't suffer. We were complimented by the computer experts that were there and told it was the smoothest turn around they have had in 12 go lives.
So anyway, be good to your crew. be supporting, i played music and brought chocolate.

GOOD SAVES, BAD SAVES
SAVING LIVES, AND FEELING GOOD ABOUT IT, BAD SAVES.
Feeling good-----2y/o female chokes on a bean, she’s blue, we fix her in less than 5 min, she comes back to visit the next day. She’s alive and well--good save.
A 40 y/o male with no history is having a heart attack, we do everything right , he goes home to his family in a couple days, he’s alive and well.--good save
A 92y/o female is brought in bleeding in her brain, a lot, the family wisely says ‘just let her go’ she passes peacefully with her family around her. We feel good.
Feeling bad------a 76 y/o male is brought in in cardiac arrest, the family says ‘just save him’ we do everything we can and his heart starts beating again. There are tubes everywhere, he was down approx 45 minutes. He is alive…the family is happy….. his brain is dead. His ribs are broken. He has tubes everywhere, he will never wake up. We don’t feel very good.---bad save
32 y/o female, 9 mos preg, she collapsed at home, she has a huge bleed in her brain.she’s kept alive long enough to deliver her 1st baby. Mom is dead, baby is beautiful. Mixed feelings on this one.
5 drowning victims in one week - 2 are dead, 2 will never be ok again, one made it thru ok. All are under 3 years of age. 3 of them were being watched by a sibling less than 7 years old. Guess which 3. One set of parents doing the right thing. They just bought the house and on the 1st day in, they were putting up the safety fence around the pool and went to answer the phone. (I turned my back for only a couple minutes) she didn’t make it. feeling bad.
LIFE IS REALLY JUST NOT FAIR, WE HAVE ALL THESE PEOPLE WHO ARE NON-COMPLIANT WITH MEDS AND INFO. THEY HAVE ALL THE BAD STUFF. DIABETES, HTN, CHOLESTEROL, ETC. WE GET UPSET AND TRY TO TELL THEM TO DO WHAT THE MD'S TELL THEM. ----THE POINT HERE????THEY FRIGGIN' LIVE FOREVER. (NOT THAT I WANT SOMEONE TO DIE)-----------

AND THEY SHOW UP IN FRONT OF THE ER AND SHE'S ALREADY FUCKING DEAD, OR AS CLOSE AS YOU CAN GET ANYWAY. JUST GETTING HOME FROM A VACATION. DOESN'T FEEL GOOD, THE TAXI IS REROUTED TO THE HOSPITAL AND SHE FUCKING GOES DOWN 10 MIN BEFORE REACHING US. ----WE PULL HER OUT AND DO EVERYTHING WE CAN POSSIBLY DO. BUT NO AIR FOR 10 MIN, IS NOT GOOD. BUT WE FUCKING TRY. THEN THE HUSBAND SAYS TO STOP. THERE IS NO QUALITY OF LIFE LEFT. HE MADE THE CORRECT CHOICE, BUT NOW HE HAS TO BURY HIS 2ND WIFE.

GET AN ADVANCED DIRECTIVE AND TAPE IT TO YOUR CHEST. GET IT TATTOO'D. THE STUFF WE DO TO SAVE YOUR LIFE FUCKING HURTS. AND WE WILL DO IT. THE LAW SAYS SO. WE WILL BREAK YOUR RIBS, PUT TUBES EVERYWHERE. STICK YOU WITH THE BIGGEST NEEDLES. GIVE YOU ALL OF THE DRUGS. AND BASICALLY MAKE YOU A FRESH CHOICE SALAD BAR, JUST TO SAVE YOUR LIFE.. RUDE DOCTORS
So.....for anyone who actually works in an er, you know how the doctor thing works. We ''live'' with them. They don't just pop in from time to time, they have to interact with us for entire shifts.
At our place, we love our docs. Of course there are times when we disagree or there is a personality clash, or they bark a little about why something isn't done, but for the most part we all get along, we go to some of the same parties, or breakfast/drink after a shift. We tell them the same jokes, they become our primary care mds because we trust them.
There is another group of md's that come in to consult pts. They are from all services, cardiology, ortho etc. the most prominent are the HBS (hospital based doctors) they do most of the admitting. the er docs do not admit at our place, if they determine someone needs to stay in the hospital, they call HBS for consult.
Well. they absolutely think their shit doesn't stink. there is a small group that knows how we work and work WITH us instead of AGAINST us.
Some of these guys and or ladies are full on assholes. they really think that we are there to serve them. we do let them know we have other pts and will get to it as soon as we can. They order stuff that could be done when the pt gets upstairs to the floors. But then they would have to go up and follow the pt. and we do get things done faster in the er. But they clog our already busy system.
The funny thing is, if they were nice about it, we would be happy to do it. Some of the newer nurses jump when they bark, I fix that real quick.
And some of the docs are just stupid, you wonder how they made it thru medical school. Some truly think their shit doesn't stink. For example:
one of the stupid ones----she writes her floor orders and doesn't 'stamp' (put the pts name on them) and just puts the orders in the pts chart. How the fuck do we know that's the right pt. I found out my unit assistant was stamping them.. A non medical person was taking the responsibility that the orders actually belong to the pt that the orders are sitting on.
So, I stopped the md one day and asked her to stamp her orders. she went ballistic. I explained that by stamping them that she is assuring that the right pt, right orders etc. that they really belong together. It will eliminate errors and the idea that my UA doesn't have to go to court because the pt got the wrong med or test. she argued for approx 5 min, then said ''you guys are wasting my time, I am not a secretary'' so I ask her, ''What makes you think your time is more valuable than ours, and if you had just done it, no time would have been wasted''
Anyway, this is an easy one. we try to do everything we can for the pt. but sometimes you have to stop and stand up for yourself or your crew.
I tend to get in trouble a lot.
but fuck them, we have to work together.

LAZY NURSES
Ok, we all have lazy moments, or days you just don't want to go above and beyond. Maybe the weather is beautiful and you just wish you were at the beach or maybe you had a bad day at home, kids sick, dog shit on the carpet, whatever. Just not motivated. Of course you do your job and take care of people, just not with the usual spunk.
Could be you are a little burnt out. All we see are sick people. could get depressing. For whatever reason, just not up to snuff.
Then there are the chronically lazy nurses, good nurses mind you, nurses who are better suited to one to one care or ICU. In a very busy er with people waiting hours to get a bed, these nurses drag the entire dept down, the flow becomes non-existant.
Every nurse has a 4 bed assignment, and we have a few that demand extra help (see above post) and others that try to manipulate the system by not letting anyone know if a pt was d/c'd. They close the curtain, leave the chart in the rack, put the flags down. Prior to d/c they go and get people a meal (not that I don't want to feed people), they send the families to the pharmacy to pick up the prescriptions, etc.
All the while you are looking for a bed for a pt that is really sick. You have the triage rn's start care in the waiting room. You have ambulances coming in and are loading people in the hallways.
Keep in mind I want people to be happy with their care, BUT, this is not a day spa, I have 15 people waiting for beds.
Last night, due to sick calls and traing for the new computer system, I had 3 such nurses that were filling in for others. There comes a point where I would rather work short that have people that sink the dept.
All evening long I was calling them on their shit. Every excuse in the book were the answers to my questions. All the way to out and out lies.
Well my answer to these nurses and other nurses out there is FUCK YOU. We have a job to do and are suppose to expedite care.
Some of these nurses work 2-3 jobs and are tired, WTF. For those of you that read about national averages in wages, what is the average for nurses, 50k? well we live in CA, near a big city. I work 40 hours a week, with little OT, and I do work every holiday (hello double time and a half) and I made 160k last year.
These nurses all work for other places too. One such nurse is an instructor 4 days a week, works 4 evenings a week for us and does 2 - 24 hour shifts as critical care transport. Well, I can do that math. He is a great one to one nurse and a nice guy, but he sucks as an assignment nures, and my thought is, does he suck at all of his jobs or just this one. Not fair to us, the pts or probably his other jobs. BTW, he is single, rents a small apt, does pay child support. What does he do with all that money? Is it worth it? Too tired to be an effective member of the team? Should I feel sorry for him. Hell no!
I work hard and almost always bring my A game to work. I have had my moments. But that's what they are, moments.

hello you drove yourself!
Let's see, I did not go over to your house and knock on the door and ask or beg you to come to the er.
Why do people, the least sick btw, think that you owe them?
The people who are really sick or potentially really sick, hardly ever say a word. They know that we are doing everything we can to expedite care. If their level of acuity should change or if someone could potentially die, i'll take someone elses bed. I really care what happens to these people and you want my crew taking care of them.
If you have the flu or a cold or cut your finger on a piece of glass or you are intoxicated or have a sunburn or you have a prescription that has run out, etc, I could go on forever. THIS IS NOT AN EMERGENCY!!!!!!!!!
These people and their families are the worst. They want us to pick them up out of their cars. (my question is how did you get them in) sometimes they get thru the doors to the treatment areas and just fucking stand there. They come up to the triage nurses every 5 min and waste the nurses time asking when it is their turn, and they have to go to work or I havn't had dinner yet, etc.
You try to explain to them nicely the flow of the er and they get angry. They often ask what other people have or state ''I know i'm sicker than that guy'' this is where I usually get into trouble.
For example: A man comes up to the triage area with his wife (she is in scrubs) states ''I can't breathe'' I check his o2 sat, listen to his lungs (clear) and smaile and try to reassure them. The wife--''he is not breathing'' I say, ''Yes , he is breathing, his vital signs are good, he will see a doctor in a little bit, please have a seat'' wife: ''How do you know?
me: ''Well, he is speaking in full sentences, his o2 is 100%, his lungs are clear, he is breathing'' This goes back and forth for a while and I ask about the scrubs she is wearing. ''are you a nurse?'' ''Yes'' ''where'' ''at the clinic'' ''an rn?'' ''yes'' me: "Then you should know he is breathing'' her: ''You don't know anything, I can tell when he is not breathing''
Again this is where I get in trouble. I start my speech ''I am a traige nurse and am specially trained to determine if an actual emergency exists, he IS breathing and you can please have a seat'' her: how do you know?'' (is she retarded and how did she graduate) me: I make more money than you, I get paid to know if someone is breathing and my thought is you should go back to school and take a breathing 101 class'' Now she is pissed, but I win.
I always try to start nice, but quite often it turns bad and you have to play the ''last word game''.
Am I rude? wtf, I know my job. and this came from another nurse.

HOW RUDE IS THIS?
So at my hospital, we pride ourselves on being welcoming to incoming nurses, including new grads, transfers, travellers etc. You never have that ''nurses eat their young'' problem at our place. It was one of the best things I liked about it when I was a new grad. Welcomed with open arms. Never set up to fail or having to 'prove myself', if i didn't know, I asked. There were so many people willing to help during that scary part of becoming a nurse. It still continues, the crew we have is welcoming and teaching, nurturing. We want these nurses to have our backs, we want to trust and be trusted.
So now to my bitch for today.
I say bring me new grads anyday! A continuing problem we have is new hires that we are reassured have 'years of er experience'.
Well that's fine and we welcome them. BUT......after their orientation, approx 75% of them turn into princesses.
Example: We just hired a nurse with 10 yrs experience from prominent, highly respected hospital. During orientation, she was sweet, smart, helpful, etc. Me, being the way I am, no holds barred, said to her, ''........, thank you for not being a princesses, I think you'll be a good fit here'' she replied, ''Oh no, not a princess, I'm as real as they get.''
Well, FUCK YOU! The day after orientation, the first thing out of her mouth is ''So...who is assigned to me?'' I asked what she meant. ''You know, who are my techs and floats?'' I said, ''What do you actually need?'' Her reply...''My pt needs a bed pan and I have paperwork to do''
I said, ''Well, the bedpans are in the storage room, they fit right under the pts ass, as for the paperwork, I think your tiara slipped, maybe you could hire a secretary''.
WTF!!!
We all treated her so well and now she is evil diva princess, that was a couple months ago and nothing has changed, trust me we have tried.
Well, paybacks a bitch. You want to play? Bring it on.
Am I rude? I don't think so. we have tried and tried. I think she needs the bitch slap not me.

Rude Nurse
Hi,
Welcome to my new blog.
First off, I truly love my job. This has nothing to do with whether or not nursing is the best job in the world, or whether I am a disgruntled employee, etc. I'm not.
I have been called 'rude nurse' more times than I care to say.
I don't call it 'rude' (could be debated at times,,,,hello, I am human).
I have this reputation at work as being 'hard' or 'aggresive'. I call it assertive. I am not afraid to speak my mind, or say what is needed. I also do not take any shit from anyone.
Just for a little bit of history, I am a swing shift charge nurse. I have a great relationship with the nurses I work with. Out of 90 rn's, techs, clerks, etc, there are probably a handful that really don't like me.
As a charge nurse, I take care of my nurses, I stand up for them, I support them, I teach them if they ask for help, I tell them when they have done a great job. I do however again, do not take shit or tolerate any that may be lazy or late or 'rude'. but I call them on their shit. I like real time corrections, good or bad. I do not expect anything from them that I wouldn't do myself. I try to make a good working environment. I play music, I bring food. I tell dirty jokes, etc. we have fun on my shift. But, we have a serious job and take that job seriously. When push comes to shove, you want my team there, we will save your life, if it can be saved.
Which brings me to my blog. It will be a multitude of things, bitch fest, tips, good stories, bad stories, and of course RUDE on occasion.
My first rude story..........
THERE IS A YOUNG WOMAN THAT COMES IN QUITE FREQUENTLY FOR PSYCH ISSUES (A SUNDAY SCHOOL TEACHER, BTW) SHE HAD PRESENTED HERSELF THE THE ER EVERY DAY FOR APPROX 4 DAYS. ON THIS DAY THE WAIT IN THE ER IS APPROX 4 HOURS TO GET A BED. WE ARE TIRED AND VERY BUSY.
WE GET THE OVERHEAD THAT A PT IS DOWN IN THE PARKING LOT. I GRAB A CREW AND A WHEELCHAIR AND HEAD TO THE PARKING LOT. LAYING ON THE GROUND IS THIS WOMAN. I'M SURE I SAID THIS OUT LOUD. ''WHAT THE FUCK!! ______ WHAT ARE YOU DOING? PT ''MY STOMACH HURTS'' (KEEP IN MIND IT HAS HURT FOR 4 DAYS, AND AFTER THOUSANDS OF DOLLARS TO TELL HER WE CAN'T FIND ANYTHING WRONG) I SAY ''GET UP OFF THE GROUND AND GET IN THE WHEELCHAIR, I AM NOT PICKING YOU UP, YOUR BEHAVIOR IS INAPPROPRIATE'' I TURNED AND WALKED AWAY, THE CREW I SENT OUT THERE MADE SURE SHE GOT INTO A WC AND INTO THE ER, WHERE SHE WAITED APPROX 3-4 HOURS FOR A BED.
DOES THE STORY END THERE? HELL NO. THIS BITCH CALLED THE HEALTH DEPT AND JAHCO AND REPORTED ME AS ''RUDE NURSE'' OUR DEPT GOT TO SPEND 2 DAYS WITH STATE WANDERING AROUND OUR DEPT, OFFICIAL WRITE UPS ETC.
WAS I RUDE? MAYBE, HAD SHE BEEN SOMEONE I HAD NEVER SEEN BEFORE, OR KNOWN HER HISTORY, IT WOULD HAVE BEEN DIFFERENT.
SHE IS ONE OF THOSE PEOPLE WHO CALL THE AMBULANCE FROM THE WAITING ROOM. PRETENDS TO NOT BREATHE ETC. YES WE HAVE 5150'D HER AND SHE IS BEING FOLLOWED FOR THESE ISSUES.

As a charge nurse, I take care of my nurses, I stand up for them, I support them, I teach them if they ask for help, I tell them when they have done a great job. I do however again, do not take shit or tolerate any that may be lazy or late or 'rude'. but I call them on their shit. I like real time corrections, good or bad. I do not expect anything from them that I wouldn't do myself. I try to make a good working environment. I play music, I bring food. I tell dirty jokes, etc. we have fun on my shift. But, we have a serious job and take that job seriously. When push comes to shove, you want my team there, we will save your life, if it can be saved.
Which brings me to my blog. It will be a multitude of things, bitch fest, tips, good stories, bad stories, and of course RUDE on occasion.
My first rude story..........
THERE IS A YOUNG WOMAN THAT COMES IN QUITE FREQUENTLY FOR PSYCH ISSUES (A SUNDAY SCHOOL TEACHER, BTW) SHE HAD PRESENTED HERSELF THE THE ER EVERY DAY FOR APPROX 4 DAYS. ON THIS DAY THE WAIT IN THE ER IS APPROX 4 HOURS TO GET A BED. WE ARE TIRED AND VERY BUSY.
WE GET THE OVERHEAD THAT A PT IS DOWN IN THE PARKING LOT. I GRAB A CREW AND A WHEELCHAIR AND HEAD TO THE PARKING LOT. LAYING ON THE GROUND IS THIS WOMAN. I'M SURE I SAID THIS OUT LOUD. ''WHAT THE FUCK!! ______ WHAT ARE YOU DOING? PT ''MY STOMACH HURTS'' (KEEP IN MIND IT HAS HURT FOR 4 DAYS, AND AFTER THOUSANDS OF DOLLARS TO TELL HER WE CAN'T FIND ANYTHING WRONG) I SAY ''GET UP OFF THE GROUND AND GET IN THE WHEELCHAIR, I AM NOT PICKING YOU UP, YOUR BEHAVIOR IS INAPPROPRIATE'' I TURNED AND WALKED AWAY, THE CREW I SENT OUT THERE MADE SURE SHE GOT INTO A WC AND INTO THE ER, WHERE SHE WAITED APPROX 3-4 HOURS FOR A BED.
DOES THE STORY END THERE? HELL NO. THIS BITCH CALLED THE HEALTH DEPT AND JAHCO AND REPORTED ME AS ''RUDE NURSE'' OUR DEPT GOT TO SPEND 2 DAYS WITH STATE WANDERING AROUND OUR DEPT, OFFICIAL WRITE UPS ETC.
WAS I RUDE? MAYBE, HAD SHE BEEN SOMEONE I HAD NEVER SEEN BEFORE, OR KNOWN HER HISTORY, IT WOULD HAVE BEEN DIFFERENT.
SHE IS ONE OF THOSE PEOPLE WHO CALL THE AMBULANCE FROM THE WAITING ROOM. PRETENDS TO NOT BREATHE ETC. YES WE HAVE 5150'D HER AND SHE IS BEING FOLLOWED FOR THESE ISSUES.
WAS I RUDE? NO, EXASPERATED, TIRED, SCARED FOR THE CARDIAC PTS IN THE WAITING ROOM, THAT'S WHAT I WAS, I HAVE NO PATIENCE FOR SHIT GAMES LIKE THIS.
